GuidedSMILE CombiGuide
Guided bone reduction followed by precise guided implant placement.
CombiGuide is a bone fixated, stackable full arch surgical guide system designed to provide physical guidance for planned bone reduction and controlled implant positioning.
Stackable full arch system
Bone fixated after tissue reflection

All guided surgical kits
Physical guidance for two critical surgical stages
CombiGuide addresses two important considerations in full arch implant surgery: achieving the planned amount of bone reduction and placing the implants in their approved positions.
The system is fixed to the bone after tissue reflection. Its stackable components provide a physical reference for bone reduction and controlled guidance for drilling and implant placement.
The standard CombiGuide uses a metal fixation base and metal osteotomy guide. It is focused specifically on guided bone reduction and implant placement.
CombiGuide is designed for:
- Full arch implant cases
- Cases requiring planned bone reduction
- Controlled guided drilling and implant placement
- Any number of implant sites
- All guided surgical kits
What is included
The CombiGuide package brings together the planning and surgical guidance required for planned bone reduction and fully guided implant placement.
Case planning video
Planned bone reduction
Included where required by the approved treatment plan.
Fully guided implant placement
Five CombiGuide components
Each component has a defined role within the surgical sequence. The fixation base remains secured to the bone while the other components are introduced as required.
Pin guide
The delivery device used to position the fixation base. It may be tooth supported or tissue supported and is removed once the fixation base has been secured.
Fixation base
The anchor that is pinned to the bone and remains in place throughout the procedure. The other guide components clip into this base.
Bone reduction guide
Establishes the planned bone reduction level required to create the intended vertical restorative space.
Osteotomy guide
Controls the drilling and implant placement sequence, including rotation indexing for the planned implant angulation.
Carrier guide
Verifies that adequate bone reduction has been completed. Passive seating confirms that the planned reduction level has been achieved.
What we need to plan your case
The exact record requirements depend on the patient’s dentition, the planned treatment and whether GuidedSMILE Grammetry will form part of the workflow.
CBCT scan
A clear CBCT scan supplied as DICOM files and captured according to the correct protocol for the patient’s dentition.
Arch records
Accurate records of the working arch and opposing arch, supplied as digital scans or the required physical records.
Bite records
An appropriate bite record or scan appliance workflow that allows the intended relationship between the arches to be established.
Clinical prescription
The planned implant sites, implant system, guided surgical kit and relevant clinical requirements for the case.
The CombiGuide surgical sequence
The components are used in sequence while the fixation base remains secured to the bone.
Reflect the tissue
Reflect the tissue to expose the bone required for fixation and the planned reduction.
Seat the pin guide and fixation base
Seat the pin guide together with the fixation base, then drill and place the fixation pins.
Remove the pin guide
Remove the delivery guide. The fixation base remains secured to the bone for the rest of the procedure.
Complete the planned bone reduction
Reduce the bone to the planned level using the fixation base and bone reduction guide as the physical reference.
Verify with the carrier guide
If the carrier guide does not seat passively, complete further reduction and repeat the check before beginning the osteotomies.
Seat the osteotomy guide
Clip the osteotomy guide into the fixation base and complete the planned drilling sequence using the selected guided surgical kit.
Place the implants
Place the implants through the guide where supported by the selected system, then continue with the planned multi-unit abutment procedure.
A fixed reference throughout the digital workflow
When CombiGuide is used within a GuidedSMILE Grammetry workflow, the metal fixation base can act as a stable, scannable reference throughout surgery.
Because the fixation base remains in position, it can be captured with the existing teeth before extraction and recorded again after extraction and implant placement. These captures help preserve the spatial relationship between the pre-surgical bite and the implant-level records.
GuidedSMILE Grammetry has its own record-capture and restorative protocol. The required workflow should therefore be confirmed during case planning.

CombiGuide FAQs
Practical information about the guide system, its components and its clinical application.
What is GuidedSMILE CombiGuide?
CombiGuide is a bone fixated, stackable full arch surgical guide system that provides physical guidance for planned bone reduction and controlled implant placement.
What is included in a CombiGuide package?
The package includes full case restoratively led planning, a case planning video, planned bone reduction where required and fully guided implant placement.
What components make up the system?
CombiGuide consists of a pin guide, fixation base, bone reduction guide, osteotomy guide and carrier guide.
What does the carrier guide do?
The carrier guide verifies whether adequate bone reduction has been completed. Passive seating confirms that the planned reduction level has been achieved. If it does not seat passively, further bone reduction is required.
How many implant sites can be included?
CombiGuide can support any number of implant sites and can be planned for all guided surgical kits.
Can CombiGuide be used with GuidedSMILE Grammetry?
Yes. The metal fixation base can provide a fixed, scannable reference that remains in place during the surgical and digital record-capture stages. The separate GuidedSMILE Grammetry protocol and patient records are also required.
What is the difference between CombiGuide and CombiGuide Lite?
Standard CombiGuide uses a metal guide system and includes a case planning video. CombiGuide Lite replaces the metal guide with an economy resin guide and includes a PDF plan without a case planning video.
